Short Course

Course Overview

Understand the principles and practice of effective safety investigation and analysis and how to achieve better system safety.

 

Co-designed by academic and industry leaders, this course will improve your safety investigation skills, including:

  • knowing when to investigate
  • collecting evidence, including conducting interviews
  • analysing evidence using multiple tools, including the Bowtie and Accimap
  • documenting and communicating findings.
     

Aligned with RIIWHS301E – to be accredited from July 2023.

Course Content

What you'll learn

  • Understand how to use an investigation process to examine unwanted or unexpected events in work activities, including incidents and accidents.
  • Develop internal skills for collecting and analysing evidence and identifying incident trends.
  • Understand the value of personally committing to managing safety investigations and setting accountability.
  • Know how to make and implement recommendations for enhancing safety.
  • Identify new opportunities for continuous improvement.

 

Course curriculum

Introduction to safety investigation and analysis

  • Why and when to investigate
  • Legislation
  • Local policies and procedures
  • Summary of current research

How to scope and plan an investigation

  • Planning your investigation to identify the scope, resources and more.

Gathering evidence

Overview of gathering methods of evidence, including:

  • interview techniques
  • storage of evidence.

Analysis of evidence

Overview of analysing the evidence, including the use of:

  • Bowtie
  • Accimap.

Findings, recommendation and actions 

  • Overview of how to document your findings and develop an action plan.

Communication

Overview of communication, including:

  • writing the investigation report
  • communication of findings.

Additional Details

Time commitment
This is an in-person course that will require roughly 25 hours to complete.

  • 5 hours of study using online materials
  • 2-day workshop
  • 4 hours to complete the final assessment.

 

Assessment
You must complete 3 assessments during the workshop to demonstrate that you have acquired adequate incident investigation and analysis skills, including:

  • 2 written assessments documenting an incident investigation
  • 1 online quiz.


Certification and accreditation
Students who successfully complete the course will receive a Certificate of Completion to verify their skills and achievements.

Prerequisites

Who should attend
Managers, supervisors and professional staff who conduct safety investigations, including:

  • mine or project-level general managers, mine managers, section managers, superintendents, mine captains, plant supervisors, senior technical staff, safety managers and practitioners, and incident investigators
  • division or corporate technical and functional staff such as engineering, ventilation, safety, human resources, finance, and procurement
  • contractor managers.
